Welcome to the Mandated Reporter Follow Up Application!

The goal of this application is to provide a user-friendly method for mandated reporters to satisfy their legal responsibilities of completing and submitting the Department of Justice BCIA 8572 form within 36 hours of calling in a Suspected Child Abuse Report. This also applies to the completion and submission of the SOC 341 and SOC 342 forms, as authorized by the California Welfare and Institutions Code Section 15658, within 48 hours of calling in a Suspected Elder/Dependent Adult Abuse Report.

As of January 1, 2022, legislation has been enacted that changes the age of an Elder to age 60 and over and a Dependent Adult to ages 18-59. The California Department of Social Services will be updating the Elder and Dependent Adult abuse report forms SOC 341/342. Please continue to use the same forms; this website will be updated as soon as new forms are available.
